To demonstrate support of the hospital mission and vision by providing nursing, case management/disease management services to clients and families to help in the coordination and management of individuals’ health needs through the full continuum. The Registered Nurse demonstrates the ability to assess, coordinate, plan, monitor, implement and evaluate the patient’s plan of care while ensuring and facilitating the achievement of optimal quality, clinical and financial outcomes. Performs assessment of clinical and life issues for the patients of COH. The successful candidate: Must be able to work FT 7a-7p and/or 8a-8p Mon-Friday, and Sundays. Weekends and holidays are required as needed - 2 Sundays per 6 weeks.
