About The Position

Nazareth Hospital, a member of Trinity Health Mid-Atlantic, is seeking a Registered Nurse (RN). The RN is responsible for assessing, planning, implementing, and evaluating patient care delivery, and for delegating patient care to other healthcare team members. This role involves providing comprehensive care according to hospital policy, proactive communication with the healthcare team regarding patient conditions and discharge plans, and thorough documentation of assessments, plans, interventions, and outcomes. Specific documentation includes pain assessment, fall risk, and wound risk. The RN will interpret and utilize clinical data, assess patient risk factors, and participate in multi-disciplinary discharge planning. Requirements

  • BSN or BS/BA degree in Nursing
  • Current RN Pennsylvania state license
  • Current BCLS required or obtained by completion of orientation
  • ACLS required or obtained by completion of orientation if Applicable to Area of Practice

Responsibilities

  • Assessing, planning, implementing and evaluating the delivery of patient care
  • Assuming the responsibility and accountability for the delegation of patient care to other members of the healthcare team
  • Providing total, comprehensive care to patients by applying the nursing process in an organized and systemic manner according to hospital policy
  • Communicating proactively to the health care team, plan of care, changes in patient condition, the patient progress, the discharge plan and other relevant information
  • Documenting assessment/reassessment, plan, interventions and outcomes according to policy
  • Interpreting and utilizing clinical data in the plan of care
  • Documenting pain assessment and plan of care
  • Documenting fall risk and plan of care
  • Documenting wound risk and plan of care
  • Assessing patient for potential risk factors
  • Participating in discharge planning, using a multi-disciplinary approach
