About The Position

The Registered Nurse Disease Manager (RNDM) position is a fully remote role focused on performing assessments, education, and training to coordinate care for the dependents of Active Duty military personnel and eligible retirees. The role requires a high level of organization and the ability to work independently in a high-volume environment.

Requirements

• Bachelor's Nursing Degree
• At least one active state nurse license, preferably compact state licensure
• At least 2 years' experience working as an RNDM, preferably in a remote or hybrid environment
• Current certification in a disease-management related area such as Chronic Care Professional (CCP), Certified Health Coach (CHC), Certified Diabetes Educator (CDE), or Certified Asthma Educator (AE-C)
• Highly organized and self-directed with the ability to function in a high-volume environment
• Strong verbal and written communication skills
• Proficient in Microsoft Office applications and possess strong technical aptitude
• Access to secure, high-speed internet

Responsibilities

• Perform assessments and coordinate care for dependents of Active Duty and eligible retirees.
• Provide education and training related to disease management.
• Utilize strong communication skills to interact with patients and healthcare teams.
• Maintain accurate records and documentation of patient interactions and care plans.
