As a Registered Nurse for Admission and Discharge, the top priority is delivering exceptional patient-centered care.   • Obtains a nursing assessment history, and ensures proper inpatient care unit admission.
  • Takes appropriate action when needed, such as communicating special circumstances to physicians and/or care team or reporting indications of abuse or neglect.
  • Completes and facilitates the completion of required documentation, such as admission assessments, risk assessments, medication reconciliation, discharge paperwork, and post-hospital documentation.
  • Teaches patients/families/caregivers about a patient's medical condition, patient status, treatment plan, medications and possible side effects, and follow-up measures.
  • Translates complex medical terminology to ensure complete knowledge.
  • Completes extra discharge documentation, education, and discharge care for special patient populations, to ensure compliance with core measures for discharge.
  • Ensures patients/families/caregivers receive copies of discharge paperwork and any other documentation needed, such as referrals for ongoing care, educational materials, etc.
  • Coordinates with House Supervisors/Nursing Administration to prioritize workload and identify greatest need areas to ensure smooth patient transitions.
  • Demonstrates a time-conscious awareness and ability to prioritize.
  • Basic Cardiac Life Support must be obtained within 30 days of employment start date.
  • (RN) Registered Nurse.
  • Associate Degree, or Bachelors Degree, or Registered Nurse Diploma.
