About The Position

As a Registered Nurse (RN) at Summit Health, you will be an integral part of a multidisciplinary care team, providing individualized, team-based care under the direction of the Primary Care Provider, Clinical Practice Manager, and Office Supervisor. This role involves delivering high-quality patient care, coordinating care activities, and ensuring effective communication among patients, staff, and physicians. The RN will utilize their clinical expertise to assess, plan, and evaluate patient care while adhering to professional nursing standards.

Requirements

  • Associates degree required; Bachelor's degree preferred.
  • New Jersey State Nursing License and BLS certification required.
  • 0 - 1 years experience; 2-4 years experience preferred.
  • Ability to communicate in English, both orally and in writing.
  • Strong interpersonal and organizational skills.
  • Ability to perform diverse work assignments with time limitations with a high degree of accuracy.
  • Experience with Standard Office Equipment preferred.
  • Experience with EHR, Practice Management System, and Departmental/Ancillary Systems preferred.

Nice To Haves

  • Experience with patient care equipment preferred.
  • Experience with Standard Office Technology in a Windows-based environment preferred.

Responsibilities

  • Assist the physician and staff with the daily coordination of patient care and physician access.
  • Function as the liaison between patients, staff, and physicians.
  • Maintain efficient patient flow and deliver direct patient care within the scope of licensure.
  • Collect lab specimens according to departmental guidelines and accurately label specimens.
  • Perform Point of Care testing, EKG's, and phlebotomy as applicable.
  • Prioritize patient care by implementing physician orders and nursing procedures.
  • Ensure proper preparation for patient visits and manage Nurse Visit schedule.
  • Perform patient intake, check-in/check-out as needed.
  • Effectively communicate problems, concerns, or issues to the Office Supervisor/Manager.
  • Obtain vital signs accurately and completely as required.
  • Document appropriately in EHR and administer injections according to appropriate protocol.
  • Perform patient outreach to satisfy quality measures (BP checks, vaccinations).
  • Coordinate and facilitate daily huddles and demonstrate telephone triage protocols.
  • Schedule appointments appropriately and accurately according to department guidelines.
  • Monitor daily hospital discharge list and collaborate with Care Management for TCM appropriate visits.
  • Review and update medication list in EHR and initiate prior authorization requests.
  • Facilitate transition of care to UCC/Hospital and maintain proficiency with department-specific equipment.
