How you’ll make an impact in this role Patient-Centered Care Coordination: Implement and monitor individualized care plans tailored to unique clinical needs, ensuring all interventions align with the patient’s personal goals and recovery strategy. Interdisciplinary Collaboration & Advocacy: Serve as the primary liaison between patients, families, and the care team to facilitate seamless communication and advocate for high-quality, aligned care. Clinical Monitoring & Execution: Provide high-standard clinical support by accurately assessing health status, documenting changes, and executing physician orders with precision to support the overall treatment strategy. Continuity of Care & Discharge Planning: Lead comprehensive discharge and transition efforts, empowering patients and their support networks to move confidently toward the next stage of healing.

Requirements

  • Advanced Life Support obtained within 6 Months (180 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
  • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
  • Licensed Registered Nurse credentialed from the Tennessee Board of Nursing OR current home state license for multi-state license recognition "Compact State" obtained prior to hire date or job transfer date required.
  • Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
