About The Position

The Complex Care Center (CCC) provides comprehensive primary care for children with complex chronic medical conditions at our Burnet and Liberty campuses. Our patients often have multiple diagnoses and may be technology-dependent, requiring supports such as home ventilation or tube feedings to maintain daily function and quality of life. We care for patients from birth through adolescence—supporting families with everything from acute illness visits to routine check-ups and immunizations—while delivering highly coordinated, multidisciplinary care. The Registered Nurse II in the Complex Care Center delivers high-quality, patient- and family-centered care to a medically complex pediatric population in a non-acute, ambulatory setting. This role requires strong clinical judgment, care coordination expertise, and the ability to manage patients with multi-system conditions and medical technologies.

Requirements

BSN from ACEN, CCNE or NLN CNEA accredited institution or MSN.
5+ years related experience is required.
Active Ohio RN License.
May be required to obtain other state licensure.
Active certification and some require specialty certification
Strong background in managing medically complex pediatric or similar patient populations

Nice To Haves

Recent inpatient critical care experience
Experience in areas such as: Complex Airway, Transitional Care Center, Emergency Department
Ambulatory clinic experience with similar patient populations, including: Tracheostomy care, Gastrostomy tube (G-tube) management, Multi-system, medically complex conditions

Responsibilities

Provide comprehensive nursing care to pediatric patients with complex, chronic conditions
Perform clinical assessments, triage, and care coordination across multidisciplinary teams
Support patients with tracheostomies, ventilators, G-tubes, and other medical technologies
Educate patients, families, and caregivers on disease management and home care needs
Collaborate with providers and specialists to ensure continuity of care
Participate in quality improvement initiatives and evidence-based practice
Office duties, including paperwork and form completion, phone triage
Demonstrates consistent integration of the Interprofessional Practice Model (IPM) in all aspects of practice.
Synthesizes and interprets multiple sources of data; uses past experiences to anticipate problems; seeks interprofessional collaboration; recognizes and responds to complex situations.
Leads efforts to improve documentation of the plan of care, care delivered, and outcomes of care.
Has innate awareness of the patient and family's current and future needs, and fully engages them in the plan of care.
Collaborates with family, other health care providers and community resources to develop comprehensive plans of care.
Caring practices follow the patient's and family's lead; orchestrates processes to ensure patient/family's comfort and concerns are addressed.
Promotes a healing environment for all health outcomes, including death.
Expertly integrates age specific concepts into patient care, considers both the patient's chronological age and developmental level.
Using self-management strategies, develops care goals and actions in partnerships with patients and families.
Creatively modifies or develops patient and family education programs.
Integrates patient/family education throughout delivery of care; evaluates effectiveness using teach back methods and by observing behavioral changes related to learning.
Integrates strategies in the plan of care that support the patient's and family's transition to home.
Sustain a working understanding of regulatory requirements and accreditation standards.
Support leadership in Compliance efforts to meet and sustain the regulatory compliance efforts of the department and hospital.
Be mindful and judicious in monitoring safety and departmental policies and procedures.
Ensure all individual certifications and Safety College requirements are fulfilled in a timely basis.
Work with department managers, nurse council and magnet representatives to identify and share safety best practices.
Be mindful of CCHMCs processes and compliance with CMS standards and freely communicate safety and compliance concerns to leadership team members.
Consistently integrates age specific and culturally inclusive concepts into patient care, taking into consideration both the patient's chronological age and developmental functioning.
Knows when and how to negotiate and navigate through the system on behalf of patients and families through care coordination.
Intervenes and leads others to change systems and processes as appropriate to support optimal patient outcomes.
