About The Position

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply. CTVHCS is in the process of making a cultural change, shifting from a disease-focused approach to a health and wellbeing approach, in order to implement the Whole Health System of Care. This is a VA-wide initiative to provide a comprehensive and systematic approach to improve healthcare delivery at all stages of Veteran care. This entails moving from an episodic, "siloed" approach to an interdisciplinary, holistic approach, emphasizing self-care in the larger context of well-being. All staff in this facility will be part of this process, shifting the focus from the question, "What's the matter with you?" to the question, "What matters to you, and how can we help you live your best life?" This strategy also involves integrating a full range of conventional approaches with Complementary and Integrative Health (CIH) approaches in order to optimize lifelong health and well-being, as defined by the Veteran, rather than narrowly focusing on disease management. The VA Community Living Center (CLC) is a nursing home setting, with a mission to restore the Veteran to the highest practicable level of well-being, maximum function and independence, prevent decline in health, and provide comfort at the end of life. VA CLC teams identify and treat Veterans' human responses to health and illness in a compassionate manner. The CLC Registered Nurse is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Administers medications and procedures per established policies and guidelines.

  • 1 full and unrestricted license from any US State or territory

Responsibilities

  • Responsible for providing competent, evidence-based care to assigned residents and oversight of licensed vocational nurses/nursing assistants, as appropriate to the setting.
  • Assist in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization consultation.
  • Demonstrates leadership in delivering and improving holistic care through collaborative strategies with others.
  • Provides peers with informal constructive feedback for improvement.
  • Fosters a safe and supportive environment conducive to the professional development of healthcare professionals.
  • Contributes professional nursing perspective in discussions with the interdisciplinary team.
  • Partners with others to effect change and produce optimal outcomes.
  • Supports colleagues through knowledge sharing to provide safe, quality nursing care.
  • Shares educational findings, experiences, and ideas with peers.
  • Questions clinical practices for the purpose of providing evidence-based care.
  • Participates in activities and strategies to sustain an evidence-based-practice culture.
  • Collaborates with the Minimum Data Set (MOS) coordinator to ensure documentation is accurate and reflective of the Resource Utilization Group (RUG) score.
