About The Position

Join our compassionate team of professionals at the Fort Meade VA Community Living Center - where Veterans receive nursing home level of care including help with activities of daily living - skilled nursing - and medical care. This is an OPEN CONTINUOUS ANNOUNCEMENT. This announcement will remain open until the closing date or until all positions have been filled. Qualified applicants will be reviewed and referred to the hiring manager on a weekly basis.

Requirements

  • English Language Proficiency
  • Graduate of a school of professional nursing approved by the appropriate accrediting agency
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth of the United States
  • Possession of a current, full, active, and unrestricted registration for graduates of foreign schools of professional nursing
  • Credit for foreign nursing education higher than associate degree/entry level requires a formal degree equivalency validation
  • Ability to participate in a pre-employment examination or evaluation

  • Experience in a nursing home or long-term care setting
  • Leadership experience in nursing

Responsibilities

  • Conduct comprehensive health assessments to identify patient needs and guide individualized care
  • Maintain accurate and timely documentation of care, progress, and outcomes
  • Monitor patients' vital signs and overall health, responding quickly to changes in condition
  • Develop and implement personalized care plans in partnership with patients and families
  • Administer medications, monitoring effectiveness, and evaluating for side effects
  • Provide skilled wound care and management to promote healing and comfort
  • Educate patients and families on health conditions, treatment options, and self-care strategies that encourage independence
  • Collaborate with patients and families to establish realistic goals, plan care, and support safe discharge transitions
  • Document teaching interventions and education outcomes in the medical record
  • Coordinate nursing referrals, including those related to discharge needs, to ensure continuity of care
  • Work closely with the multidisciplinary healthcare team to deliver holistic and coordinated care
  • Identify care needs requiring interdisciplinary support and partner with colleagues to address them
  • Participate in the development, implementation, and improvement of policies, procedures, and guidelines
  • Lead efforts to enhance patient safety, satisfaction, and overall experience
  • Act as Charge Nurse when needed, including managing daily assignments, coordinating coverage, delegating tasks, reviewing orders, contacting MOD, and promoting teamwork
