About The Position

The Registered Nurse (RN) Community Care Clinical Consultant is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. S/he will assume responsibility for the coordination of care focused on patient education, self management, and customer satisfaction throughout the continuum of care. S/he will follow procedures per established policies and guidelines.

Responsibilities

• Assess the needs of Veterans described in Community Care referrals and determine where the service can be provided according to established guidelines, keeping the Veterans' unique needs central to the provision of care.
• Utilize the Delegation of Authority from the Chief of Staff Office for authorizing care for Veterans in the community.
• Provide clinical coordination and case management for Veterans authorized for care in the community until episode is complete.
• Review incoming requests from community providers for secondary authorization of services, determining efficacy of requested services, and authorizing requests according to established guidelines.
• Serve as liaison between community providers and VA providers, when warranted.
