About The Position

This position is eligible for the Education Debt Reduction Program , a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply. The Collaborative Care Manager, Mental Health Outpatient Clinic Nurse is a Depression Care Manager that considers all characteristics of the individual in order to assist each patient with the improvement of his/her health status to the maximum potential. The Nurse utilizes elements of performance including knowledge and active participation in unit level quality improvement processes. He/she promotes the medical center mission, vision and values and complies with HRO and the DCVAMC SOPs, policies and procedures.

Requirements

  • Licensure: 1 full and unrestricted license from any US State or territory

Responsibilities

  • Responsible and accountable for individual nursing practice and determines the appropriate delegation of tasks consistent with the nurse's obligation to provide optimal care.
  • Growth and development in practice with increased levels of self-direction.
  • Ability to formulate a plan of care based on nursing assessments.
  • Delegates elements of care to the appropriate individual based upon assessed needs, condition, and safety of the patient.
  • Brings principles and theory with limited application of critical thinking skills.
  • Biopsychosocial concepts, cognitive skills, and technically competent practice in providing care to patients with simple needs on the assigned unit where conditions and treatment are normally of a noncritical nature.
  • Manage a safe therapeutic environment for patients with mental health disorders.
  • Demonstrating technical skills in assessing depression, suicidal intent, aggressive/assaultive behavior and unpredictable behavior and providing nursing care for comorbid medical conditions.
  • Provide patient care by assessing, planning, and implementing the nursing care of mental health patients.
  • identify issues, analyze, develop and implement interventions.
  • Responsible, and accountable for all elements of the nursing process when providing direct patient care.
  • Provide patient and family education, psychiatric and medical triage of patients, preventative health screenings, administration of immunizations/medications and crisis intervention.
  • Adhering to all Medical Center Policies, Nursing Service Policies, or any other document approved by the Washington DC VAMC which provides guidance on the practice of nursing within the facility.
  • All other duties as assigned as determined by the Facility, Department, and/or Agency.
