How you’ll make an impact in this role Lead Personalized Wellness Journeys: Design and monitor tailored care plans that address the unique nutritional and health needs of mothers and children, ensuring they meet vital developmental milestones. Coordinate Holistic Family Support: Act as the primary bridge between various health disciplines to ensure families receive well-rounded, seamless care that extends beyond the clinic walls. Translate Clinical Guidance into Care: Execute healthcare orders with precision to provide immediate support and education that directly improves family health outcomes. Advocate for Long-Term Success: Evaluate and facilitate "next steps" for every participant, connecting families with the community resources and follow-up care they need to thrive after their visit. What minimum requirements you’ll need Licensure / Certification / Registration: BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. Licensed Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required. Education: Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required. What additional preferences we're seeking Willing to also consider candidates with experience as a Nutritionist or a Registered Dietitian. Requirements
