How you’ll make an impact in this role Lead the implementation and continuous monitoring of personalized patient care plans to ensure every individual receives treatment tailored to their specific recovery goals. Coordinate multidisciplinary teams as the primary point of contact, ensuring seamless communication across all clinical specialties for holistic patient support. Advocate for patient safety by diligently executing physician orders and maintaining precise records of the care journey to uphold the highest standards of quality. Assess and facilitate comprehensive discharge strategies, collaborating with the healthcare team to ensure patients have a safe, well-supported transition home. What minimum requirements you’ll need Licensure / Certification / Registration: BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. Licensed Registered Nurse credentialed from the Texas Board of Nursing required. Education: Diploma from an accredited school/college of nursing required OR a bachelors level degree not in nursing, an active RN License, and enrolled in an Alternate Entry Masters of Science in Nursing Program with a minimum of 1 year in the program completed. What additional preferences we're seeking Clinical Expertise: Seeking a Registered Nurse with 3–5 years of professional experience, ideally within an outpatient or rheumatology setting. While infusion experience is preferred, we are open to training a candidate who demonstrates a strong willingness to learn. Patient & Administrative Support: Proficiency in providing comprehensive patient education and navigating the insurance authorization process for specialty medications and diagnostic imaging is essential for success in this role.
