Administration of Human Resource Programs

The CLC Admissions Coordinator reports directly to the Chief Nurse for Geriatrics & Extended Care. The CLC Admissions Coordinator collaborates with the members of the healthcare system and stakeholders to provide a seamless, timely referral system to Veterans seeking admission to the CLC to include each referral being screened and reviewed for disposition and planning.

  • Collaborates with both referring and accepting staff ensuring that Veterans are placed in the program area that can best meet their needs.
  • Collects and maintains data that will enhance program planning and placement.
  • Initiates consultation for complex administrative and clinical issues.
  • Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex resident care.
  • Assists with planning assessments, identification of barriers, and activities required to successfully transition the Veteran into the CLC setting.
  • Monitors bed availability and serves as the point of contact for the outcome of screening decisions.
  • Coordinates and collaborates with other VA and non-VA departments, community agencies, and healthcare providers to ensure Veteran needs are addressed.
  • Provides peers with formal or informal constructive feedback for improvement.
  • Possesses a basic understanding of clinical treatment modalities, organization and administration of the VA healthcare delivery systems, educational principles, clinical information systems, accreditation and regulatory standards, and program development.
  • Demonstrates proficient clinical practice skills, problem-solving skills, and decision-making skills.
  • Exhibits positive effective communication skills and professional behaviors that promote cooperation and teamwork with internal and external customers.
  • U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
  • Selective Service Registration is required for males born after 12/31/1959.
  • Must pass pre-employment physical examination.
  • Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
  • Participation in the Coronavirus Disease 2019 (COVID-19) vaccination program is a requirement for all Veterans Health Administration Health Care Personnel (HCP).
  • Graduate of a school of professional nursing approved by the appropriate accrediting agency.
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth of the United States.
  • 3-5 years successful nursing experience in med-surg areas and/or Long-term Care.
