Ambulatory Health Care Services

Under the direction of a Nurse Manager or Director of Nursing and in collaboration with administrative and clinical leadership, provides leadership and direction to licensed and non-licensed staff in the provision of nursing care to individuals with mental and/or developmental disabilities in Center service settings. Acts as role model and resource person and can effectively implement the nursing process under the requirements of the Texas Nurse Practice Act. Demonstrates effective utilization/deployment of nursing staff, encourages professional growth among nursing staff, and ensures delivery of quality nursing service. Scope of responsibilities encompasses unit/program during assigned shift, or hours of operation in outpatient settings.

  • Utilizes a five-step systematic process (nursing process) to deliver consumer services including assessment, analysis, planning, implementation, and evaluation.
  • Demonstrates decision making that is clinically sound, systematic, timely, researched and reflects awareness of impact including reporting to appropriate members of the health care team.
  • Performs technical nursing skills within the scope of practice for RN and in accordance with Agency policies and procedures and Nursing Operational Guidelines with focus on consumer, environmental and technical safety related to Crisis-EOU and Detox Services.
  • Documents clinically relevant information in consumer records that includes but is not limited to observations of signs, symptoms, test results, behaviors and response to medications and treatments.
  • Inform, explains and educates consumers/family and health care team as needed regarding medications, disease processes and health information at level fitting consumer/family and team members' comprehension.
  • Delegates specific tasks and supervises other RN's, LVN's and unlicensed staff in Center service settings (Crisis-EOU and Detox) in accordance with program design and structure.
  • In Charge role, supervises their assigned staff toward completing duties and assignments, including coaching, mentoring, and teaching as needed.
  • Promotes positive work relationships with staff, team members, supervisors and other disciplines and/or agencies.
  • Works independently with minimal supervision and demonstrates effective use of time.
  • Provides ethical nursing care in a confidential, caring, respectful, non-discriminatory manner promoting the principles of trauma-informed care, patient-centered focus, and Just Care Culture.
  • Works fluidly between services.
  • Performs related work as assigned, adapts to changing work requirements and complies with Agency policies and procedures.
  • Associate's degree Nurse (ADN) from an accredited college or university, with three (3) years of experience.
  • Current Registered Nurse License to practice in the State of Texas.
  • Must be able to meet the physical requirements to complete Nonviolent Crisis Intervention (NCI) and CPR training including lifting up to 12 lbs. and supporting up to 55 lbs.; bending, stooping and getting on and off the floor without assistance.
  • Must maintain a valid driver's license and automobile insurance coverage, be able to travel as needed, and be able to meet on a consistent basis the driving record requirements of the Company's auto insurance carrier if you drive your vehicle during company business.
  • Must maintain required and mandatory training requirements to ensure compliance with all State regulations and CHCS policies.
  • BSN Bachelor of Science in Nursing from an accredited university with two (2) years' experience.
  • Hourly rate range: $36.96-$40.54
  • Flexible scheduling with 3/12-hour shifts and rotating weekends.
