Adult Health – Home Care : Deliver and coordinate services to maintain the health of individuals families and communities through population-vase assessments and interventions. Functions individually or as part of the agency team to achieve agency goals. This position is full time and will be assigned to Adult Health. Under the direction of the Adult Health Nursing Supervisor, the nurse will provide professional nursing care to patients in home environment. Record patients’ histories and develops patient home care plans in conjunction with other medical professionals. Performs examinations and prescribed procedures, observe patients’ progress and records in patient records. Will provide follow-up care to patients and families after they have been discharged from a hospital or rehabilitation facility, skilled in nursing, or long-term care facility. Promotes patients’ independence by teaching patients’ and families to understand medical conditions, medications, and self-care skills. Summary of Some Examples of Essential Duties Performs difficult advanced human support work assessing, planning, providing, and coordinating public health services and programs. This position works primarily with evidence based home visiting services. Performs professional and skilled nursing to patients in the community setting, to promote, protect, and preserve physical and mental health in the community. Plans and provides direct care, makes referrals to providers, and evaluates services as part of a comprehensive Community Health Program. To provide professional and skilled nursing services to patients, as part of a comprehensive public health program promoting physical and mental health in the community. To carry out programs and provide a variety of individual, community, and/or systems-focused public health interventions to protect and preserve the health of the county's residents. Responsible for performing professional public health and nursing functions in support of one or more specialized public health programs or functions. Responsibilities may include developing, implementing and maintaining health programs and initiatives; managing medical records and related systems; providing health education for students, families, and employees; conducting home visits to deliver and monitor health treatments; conducting health evaluations; and monitoring client progress. Provides population based Public Health nursing practice. The practice is system, community and individually focused to protect entire populations from disease, illness, and injury and promotes physical and mental health. To accomplish this mission, public health nurse services balance three core government public health functions Position assignments may include but are not limited to: Maternal Child Health home visiting, WIC (Women, Infant, Children), immunizations, disease prevention and control, emergency preparedness, health education, Child and Teen Check-up outreach, Follow Along Program, environmental health, worksite wellness, quality improvement. Assesses family dynamics, client health history, home environment and current health status through interviews, observations and analysis of records and teaches those in positions of responsibility to recognize the early signs of disease and deviations from normal health and initiates appropriate action. Interprets patterns of growth and development and promotes optimum development for each individual. Understands and utilizes Public Health and the Public Health Nursing philosophy and knowledge and applies primary, secondary and tertiary prevention measures.
