About The Position

Under the direction of the Nursing Supervisor, this position involves providing comprehensive, person-centered, team-based, community treatment for children, adolescents, and families in the Comprehensive Community Services program to persons diagnosed with mental illness or substance abuse or dependence. Responsibilities include service facilitation, symptom management, medication management/med set up, physical health monitoring, psychosocial rehab, and skill development according to individualized Recovery plans. Additionally, supportive nursing services and medication set up for adults in the Rock County Outpatient Clinics and the Crisis Stabilization Unit are also part of the role. The position requires collaboration with a treatment team, other HSD employees, families, and collateral agencies, while supporting and promoting the Mission, Vision, and Guiding Principles of the Human Services Department.

Requirements

  • Associates Degree in Nursing (ADN) Required.
  • Bachelors Degree in Nursing (BSN) Preferred.
  • Valid Wisconsin license to practice as a Registered Nurse Required.
  • Knowledge of the current principles and practices of nursing.
  • Knowledge of assessment and treatment of persons who have mental illness including co-occurring disorders.
  • Ability to maintain a compassionate and caring attitude, maintain good professional boundaries.
  • Ability to embrace and nurture a person-centered, recovery-oriented framework and culture for service delivery.
  • Knowledge of the laws, regulations and practices pertaining to DHS 36, DHS 35 and DHS 34.
  • Knowledge of psychotropic medications, their effect, side effects and abuse potential.
  • Knowledge of community resources and how to effectively utilize them.
  • Ability to plan and organize work effectively.
  • Ability to maintain accurate current records and prepare clear and concise reports.
  • Ability to establish and maintain effective working relationships with agency staff, community members, families, and collateral agencies.
  • Knowledge of various cultures and diverse populations and ways in which cultural values impact individuals, families, and the community.
  • Ability to make assessments and provide or refer for appropriate treatment.

Responsibilities

  • Medication Management - setting up of medications, taking and filling orders, medication reconciliation after hospitalization, medication packet reconciliation each week, monitoring of medication efficacy and side effects.
  • Physician supports - interim medication checks, primarily with the Child/Adolescent Psychiatrist, side effect monitoring, efficacy monitoring, collateral contacts with primary medical doctors, coordination of medical treatment, review of laboratory reports, coordination of lab orders, injections, lab draws.
  • Nursing assessment - monitoring vitals, monitoring weights, patient education, management of mild medical issues, provide direction on when to seek ER/urgent care/PMD appointment.
  • Facilitate consumer groups and provide education related to health and medication.
  • Supportive nursing activities provided to the Rock County Outpatient Clinics and Crisis Stabilization Unit including medication management, physician support, and nursing assessment.
  • Manage a case load and provide or arrange for provision of all needed services for each person on that case load.
  • Develop and implement clinically relevant recovery plans ensuring that services are culturally competent, recovery oriented and involve consumer participation.
  • Provide interventions including assessment, service planning, service facilitation, medication management, physical health monitoring, skill development, employment related skills training, psychoeducation, and recovery support services.
  • Advocate for and arrange transition to the least restrictive living situation for all CCS consumers.
  • Act as a team member on the CCS treatment team contributing to the functioning of the treatment team.
  • Regularly assess consumer mental status, areas of functioning, achievement towards stated goals, and symptom patterns.
  • Actively participate in treatment team meetings, individual and group supervision meetings, and treatment plan meetings.
  • Maintain prompt communication with family members, team members, Rock County Crisis Intervention, and collateral agencies.
  • Documentation of treatment progress notes for CCS in accordance with DHS 36, Medicare/Medicaid.
  • Documentation of treatment progress notes for Outpatient and Crisis Stabilization supportive services in accordance with DHS 35, 34 and Medicaid/Medicare.
  • Other duties as assigned.
