About The Position

The Reg Nurse CC is responsible for providing professional nursing for patients following established standards and practices. Assists physicians and staff with the daily coordination of patient care and patient access. Coordinates patient flow, schedules appointments, procedures, tests and referrals. May serve as preceptor for clinical staff. Relies on experience and judgment to accomplish responsibilities. Works under minimal supervision. Creativity is expected to perform job. May lead and direct the work of others within scope of practice.

Requirements

• Graduate of accredited school of nursing.
• Current Louisiana State license as RN.
• Proficient in the English language, verbal and written communication skills.
• Computer skills.

Responsibilities

• Perform patient psychosocial/physiologic assessments that integrate changing data.
• Formulate a plan of care based on trends of similar patients, including patient education and discharge planning.
• Tailor and prioritize caring practice to individual needs, including cultural/ethical/spiritual needs.
• Provide active participation in the planning of routine transitional health care needs.
• Respond immediately to changes in patient's needs and provide holistic care using independent clinical judgment.
• Adapt planned educational programs and information to individual patients and families.
• Ensure efficiency in the delivery of care and determine immediate priorities.
• Advocate for patient rights and seek resolution for potential conflicts.
• Identify variances from expected outcomes and revise the plan of care based on patient outcomes.
• Accurately document all patient care activities per documentation standards.
• Communicate and collaborate with health care team members, patients, and families to maximize patient outcomes.
• Delegate patient care activities appropriately to other health care team members.
• Maintain knowledge regarding unit initiatives and incorporate outcomes into practice.
• Participate actively on unit teams, work groups, and/or committees.
• Serve as a clinical resource to health care team members and provide mentorship.
• Participate in quality improvement initiatives and incorporate evidence-based practice into nursing practice.
• Complete other duties as assigned and act as a backup for others in the department.
