This role is for a Registered Nurse Case Manager located at the Ascension All Saints Spring Street Campus in Racine, WI, within the Case Management Department/Specialty, with a schedule of Days | PRN.

Requirements

Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required.
Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.
Diploma from an accredited school/college of nursing required.

Nice To Haves

BLS Provider credentialed from the American Heart Association (AHA) preferred.
Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.

Responsibilities

Lead the creation of personalized care plans by integrating patient values and clinical needs to ensure a seamless recovery journey from admission to discharge.
Advocate for your patients by identifying and removing barriers to care, ensuring they have the resources and support needed for a safe transition home.
Drive clinical excellence by evaluating medical necessity and documentation, ensuring every patient receives the right level of care at the right time.
Collaborate with multidisciplinary teams and payers to optimize benefits and coordinate post-acute services, acting as the vital link between the hospital and the next phase of healing.
