People live, and work happy here! #IND23HH Position Summary The RN Case Manager plans, organizes, and provides home health care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities. In this role you will:Conduct assessments and complete client visitsCoordinate care alongside therapies & client/families and coordinate, supervise, monitor, and develop care. Communicate with physiciansPerform teaching to clients and families Complete complex nursing tasks within the home setting

Requirements

  • Possess a valid RN License from the appropriate state. This shall include required CEU's
  • Minimum 2 years of nursing experience
  • Home Health experience required

  • Oasis experience (Preferred - will provide training)

Responsibilities

  • Conduct assessments and complete client visits
  • Coordinate care alongside therapies & client/families and coordinate, supervise, monitor, and develop care.
  • Communicate with physicians
  • Perform teaching to clients and families
  • Complete complex nursing tasks within the home setting
