Home visits are scheduled MondayFriday, 8:00 AM4:30 PM, with no on-call requirement, and a one weekend a month rotation. Requirements

Current NYS Registered Nurse license (from an accredited program)
Valid drivers license, auto insurance, and reliable vehicle
Self-directed, organized, and comfortable working independently
Strong communication and computer skills
Ability to sit or stand for long periods
Ability to lift/carry 2540 lbs and assist with transfers up to 150 lbs
Comfortable with bending, kneeling, climbing stairs, and driving in all weather
Ability to use a phone, laptop, tablet, and electronic medical record
Category III bloodborne pathogen exposure (rare contact with blood/body fluids)

Nice To Haves

BSN preferred (tuition reimbursement available!)
Adult medical-surgical, home care, case management, or chronic disease management experience a plus

Responsibilities

Provide high-quality nursing care to adult patients through a combination of hands-on clinical care, assessment, education, and care coordination.
Deliver safe, patient-centered nursing care in the home
Serve as the primary case manager, coordinating care with the interdisciplinary team
Develop and implement individualized plans of care
Provide patient and caregiver education, support, and disease management teaching
Ensure accurate, timely documentation (documentation time built into the home visit)
Supervise and provide oversight of paraprofessional staff, including home health aides, ensuring tasks are performed safely and according to the patients plan of care
Maintain compliance with all infection control and regulatory standards
Stay current with clinical best practices
