About The Position

The Registered Nurse Case Manager position involves establishing partnerships with clients and their families to understand their health and independent living goals. The role requires conducting in-home visits throughout the Los Angeles area, including the San Fernando, Santa Clarita, Palmdale, and Lancaster valleys, to perform in-depth medical, health, and rehabilitation assessments. Key responsibilities include certifying levels of care, performing physical assessments, ensuring the delivery of authorized medical services, and collaborating closely with physicians and other health professionals to provide comprehensive care. The Case Manager is responsible for developing, implementing, and monitoring care plans in consultation with social work care managers, and for thorough documentation and reporting. This role also involves participating in the care management team through peer reviews to ensure quality services and documentation. Additionally, the position requires providing care management according to departmental policies, managing client health and psychosocial status, developing client-specific care plans, implementing community resource interventions, maintaining regular client contact, responding to crisis situations, and adhering to all regulations. Community relations, problem resolution, and effective interpersonal communication are also crucial aspects of the role.

Requirements

  • Requires home visits throughout the City of Los Angeles, including San Fernando valley, Santa Clarita valley, Palmdale and Lancaster.
  • Associates or Bachelors of Science degree in nursing required.
  • One year of experience required working in an acute care hospital providing care management, working with seriously ill, frail, disabled, and cognitively impaired adults.
  • Must hold a valid California Department of Consumers Affairs Board of Registered Nursing active license that is current and in good standing.
  • Must hold a valid current CPR/BLS card.
  • Copy of current automobile insurance provided annually of $15,000/$30,000 for injury and $5,000 for damage to property.

Responsibilities

  • Establishes partnership relationship with client and family/representatives to elicit goals, preferences and needs for health and independent living.
  • Conducts in home visits throughout the city of Los Angeles, in-depth assessment/reassessments covering medical, health, and rehabilitation concerns.
  • Certifies level of care determinations.
  • Performs physical assessments as necessary and interpreting clinical findings.
  • Ensures that all authorized medical services are delivered.
  • Works closely with physicians and other health professionals to assure comprehensive care delivery.
  • Develops care plans, implements and monitors services, consulting with the social work care manager.
  • Documents and completes reports as required.
  • Serves as a member of the care management team to support quality services by doing peer reviews to verify need for waiver services and quality of POT and documentation.
  • Provides care management in accordance with departmental policy, program standards and patient needs.
  • Completes assessment evaluation of a client’s health and psychosocial status utilizing all relevant information sources including the identification of issues that are specific to age, diagnosis and disabilities of the client population served, in accordance with departmental policy.
  • Participates as an active team member in care planning to develop care plans specific to the client’s needs.
  • Implements interventions and services utilizing community resources in a timely fashion in accordance with departmental policy.
  • Maintains and documents regular contact with clients in accordance with departmental policy and providing ongoing support as needed.
  • Identifies, assesses and responds to crisis situations in a timely fashion, with appropriate interventions.
  • Observes all legal, departmental, and/or hospital regulations.
  • Provides community relations by developing and maintaining positive community interactions building referral relationships in the community.
  • Recognizes and resolves problems in a timely and appropriate fashion.
  • Maintains effective interpersonal communication skills.
  • Uses open and accurate verbal and written communication.
  • Recognizes and resolves conflicts or unusual situations.
  • Maintains positive interpersonal relationships and team building within department and ancillary staff and with community partners.
  • Completes written correspondence and documentation in a timely manner.
