About The Position

Baystate Health, a nationally recognized leader in healthcare quality and safety, and home to Baystate Noble Hospital is looking for a Registered Nurse Case Manager to join their team! POSITION HIGHLIGHTS Coordinate Patient-Centered Care: Facilitate collaboration among patients, families, and healthcare teams to develop and implement individualized care plans, ensuring high-quality outcomes and improved patient experiences. Manage Transitions and Discharge Planning: Conduct assessments, coordinate care resources, and oversee safe, efficient hospital transitions while maintaining continuity of care and supporting medical management initiatives. Ensure Compliance and Optimize Reimbursement: Evaluate admissions and ongoing stays using approved criteria, communicate with payers to maximize reimbursement, and adhere to all compliance regulations and hospital policies. Additional Details and Schedule: Non-bedside employee Full-time, 40 hours per week 1st shift position NOBLE COMMUNITY HOSPITAL: This unique hospital is an 85-bed acute care community hospital helping people in the Greater Westfield community and offers direct access to world-class technology, diagnostics, and specialists. Requirements

  • Successfully passed Board of Registration in Nursing Examination and is licensed in the Commonwealth of Massachusetts
  • Current American Heart Association Health Care Provider CPR certification required.
  • Education: Diploma in Nursing
