This position provides health services to the youth of Hope, collaborating with other youth service lines to ensure optimal care for all youth. The role involves communicating health updates to parents/guardians and managing youth’s medications and treatments in both residential and school settings. Additionally, the Registered Nurse Case Manager provides ongoing health services training to residential and school staff. Key responsibilities include organizing, coordinating, and providing care coordination to youth within case assignments, implementing and continuously monitoring provider’s orders, and serving as a liaison between youth, staff, and providers. The individual will read and respond to all communication regarding youth’s medical care, collaborate with providers and all members of the youth’s team to ensure the best trajectory for the youth, perform bi-annual physical assessments, and schedule and attend medical appointments when appropriate. The role also entails auditing medication closets in residential settings for proper function and safety, including verifying narcotic counts, reordering medications, organizing the closet, and reviewing medication documentation for completeness and accuracy, while re-educating staff on processes as needed. Attendance at all meetings pertaining to youth within the case assignment (e.g., Plex Leadership, Whole Plex, IEPs, staffings, clinical reviews) is also required, along with performing all duties outlined in the Licensed Practical Nurse job description.
