About The Position

Constellation Health Services is looking for a Home Health Registered Nurse to aid in coverage for Suffolk County with some assistance in Nassau County! Constellation Health Services is seeking a Registered Nurse to join our start-up and survival team (S&S) in New York. As we expand our footprint, presentation, articulation, and a strong skill set are crucial for introducing our brand. Members of this team, depending on location, are asked to be the backbone of the site. This position is one of a road warrior and requires great organization and task management. This role would be responsible for a minimum of 2-3 admissions (starts of care) per day, 12-14 per week. Requirements

  • Current and active RN license in the state of New York
  • Minimum of 2 years of field RN home care experience.
  • In-depth understanding of certified home health.
  • Exceptional communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Compassionate and empathetic approach to patient care.
  • Basic knowledge of iOs operating system.

Nice To Haves

  • Bachelor’s degree in nursing (BSN) preferred.

Responsibilities

  • Deliver compassionate and comprehensive nursing care to patients
  • Provides skilled nursing care, including administration of medications and treatments as prescribed in the physician’s plan of care for patients in their home setting.
  • Conduct thorough assessments to identify patient needs and develop individualized care plans.
  • Manage medications and treatments as prescribed by the physician.
  • Provide emotional support and guidance to patients and their families.
  • Educate patients and families about their plan of care and disease trajectory
  • Collaborate effectively with interdisciplinary team members and attending physicians to ensure coordinated care.
  • Maintain accurate and up-to-date patient records.
  • Ability to travel up to 100% of the time.
