About The Position

The Registered Nurse Case Manager will help ensure that our clients receive safe, professional, and loving care in their homes. The Registered Nurse Case Manager will complete in-home clinical assessments, provide supervision of aides, and provide guidance and training to caregivers to ensure the highest quality of care for our patients.

Requirements

  • A Bachelor's degree in Nursing (Required).
  • Unrestricted Licensed IN Registered Nurse (RN) is required.
  • Current CPR/AED/BLS/First Aid certification.
  • One (1) year of professional experience practicing as a Registered Nurse (RN) in home health; previous case management/utilization review experience preferred.
  • Must be able to travel the assigned territory.
  • Must have an active driver's license and reliable transportation to drive throughout the day/territory.
  • Extensive OASIS experience and knowledge with a demonstrated ability to oversee and ensure compliance with OASIS guidelines.
  • Ability to effectively communicate with patients, families, and healthcare professionals.
  • Proficient in conducting physical examinations and assessing patient conditions.
  • Strong knowledge of medical terminology and pharmacology.
  • Ability to interpret medical test results and develop appropriate care plans.
  • Exceptional critical thinking and problem-solving abilities.
  • US work authorization (Required)
  • Must have your own computer and high-speed internet. We use MS Teams for regular video calls.
  • Willingness to travel: 25% (Required)

Nice To Haves

  • previous case management/utilization review experience preferred.
  • Medicare, ACHC and/or CHAP experience preferred.
  • Case management: 1 year (Preferred)
  • Home health: 1 year (Preferred)
  • OASIS: 1 year (Preferred)
  • BLS Certification (Preferred)
  • Understand and comply with Staffing Care Pro policies and procedures.
  • Maintain confidentiality as related to client information under HIPAA regulations regarding protected health information. Any disclosures of confidential information made unlawfully outside the proper course of duty will be treated as a serious disciplinary offense.

Responsibilities

  • Conduct in-person patient interviews and comprehensive physical assessments.
  • Oversee the implementation and ongoing assessment of the patient's plan of care through the management of home health aides, LPNs, RNs, and other caregivers.
  • Communicate patient conditions and collaborate with appropriate providers to deliver care when patient needs evolve.
  • Provide education to patients and families on proper home health care procedures. For example: wound care, IV administration, and medication management.
  • Work to decrease re-admissions by promoting preventative care and ensuring continuity of care.
  • Document patient information accurately and maintain medical records.
  • See patients and clients, as needed.
  • Auditing charts weekly.
  • Perform other duties as assigned.
