About The Position

Constellation Health Services seeks a motivated, independent Registered Nurse to deliver high-quality, patient-centered hospice care in home and community settings. This role is ideal for a self-starter who thrives in fast-paced environments and is ready to lead clinical operations in a growing territory (Lancaster, PA & Surrounding Area). As an RNCM you will be coordinating, implementing and evaluating care plans for patients in their homes or community settings. Ensuring compliance with critical and regulatory standards across all disciplines of Hospice Care.

Requirements

  • Active RN license in Pennsylvania or compact state
  • Minimum 2 years clinical experience (Hospice, Home Health, Case Management preferred)
  • Strong knowledge of hospice disciplines and resources
  • Willingness to travel within assigned region
  • Current and active RN license in the state of employment.
  • We are seeking individuals who either hold an RN license or are willing to obtain one in the following states: CT, NY, MA, NH, ME, TX, OH, NJ, PA, and VA.
  • Minimum of 1 year of experience in hospice or home health.
  • In-depth understanding of end-of-life care principles and practices and certified home health.
  • Exceptional communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Compassionate and empathetic approach to patient care.
  • Basic knowledge of iOs operating system.

Nice To Haves

  • Bachelor’s degree in nursing (BSN) preferred.

Responsibilities

  • Conduct patient assessments and develop individualized care plans
  • Coordinate interdisciplinary services (Physicians, Social Workers, Support Staff)
  • Educate patients and families on care plans and medications
  • Monitor progress and update care plans as needed
  • Maintain timely documentation in EMR
  • Lead territory launch, establish workflows, and support team onboarding
  • Maybe requested to support multiple roles (i.e.: Oncall, aide responsibilities, RN CM, etc) until a census of 5, then mentors incoming staff
