About The Position

The Registered Nurse - Case Manager position is a remote role that requires field visits to conduct health screenings and assessments. The primary responsibility is to manage medical care and provide health education to customers enrolled in government health care programs. This role involves data analysis for regulatory reporting and representing the company to its members.

Requirements

• Registered Nurse (RN) with 3 years of direct clinical care experience in a clinical setting, or Licensed Professional Counselor (LPC), or Licensed Master Social Worker (LMSW) with 2 years of clinical practice.
• Current, valid, unrestricted RN license in the state of operations or reciprocity.
• 3 years of wellness or managed care experience presenting clinical issues with members/physicians.
• Knowledge of the health and wellness marketplace and employer trends.
• Strong verbal and written communication skills.
• Analytical experience including medical data analysis.
• Current IL driver's license, transportation, and applicable insurance.
• PC proficiency including Word, Excel, PowerPoint, and database experience.

Responsibilities

• Conduct medical management and health education programs for customers on government health care programs.
• Gather, analyze, and provide data for regulatory reports.
• Represent the company to members during health screenings and assessments.
• Perform field visits in assigned territory to conduct face-to-face health screenings and assessments.
• Discuss medical needs with members and interface with internal staff, management, and external vendors.
