About The Position

The RN Case Manager position at Baptist Health South Florida is designed to support the physician and interdisciplinary team in facilitating patient care. The primary objective is to enhance the quality of clinical outcomes and patient satisfaction while managing the cost of care. This role involves integrating and coordinating care facilitation and discharge planning functions, working closely with the Utilization Review (UR) RN and Social Work Case Manager. The RN Case Manager is responsible for a designated patient caseload, ensuring effective planning to meet patient needs, manage length of stay (LOS), and promote efficient resource utilization.

Requirements

• Associates degree in nursing.
• Current Registered Nurse license.
• 3 years of hospital clinical experience.
• 1-3 years of hospital or payor Case Management experience preferred.
• Excellent interpersonal communication and negotiation skills.
• Strong analytical, data management, and computer skills.
• Current working knowledge of discharge planning, utilization management, case management, and performance improvement preferred.
• Understanding of pre-acute and post-acute venues of care and post-community resources preferred.
• Strong organizational and time management skills.

Nice To Haves

• BSN degree (required to complete within 5 years of job entry for certain RNs).

Responsibilities

• Facilitate collaborative management of patient care across the continuum.
• Intervene as necessary to remove barriers to timely and efficient care delivery and reimbursement.
• Apply process improvement methodologies in evaluating outcomes of care.
• Coordinate communication with the healthcare team, patients, families, and post-acute care services.
• Conduct readmission screening and prevention.
