The Registered Nurse Case Manager (Hospice) is responsible for identifying and coordinating patient/family care for terminally ill patients in all hospice settings. This role involves utilizing teaching, assessment, and intervention skills to provide comfort care at the end of life and maximize the quality of life for patients and their families. The Case Manager educates patients and families on disease processes, hospice philosophies, and signs of decline, while continuously reviewing patient caseloads to ensure appropriate utilization of services and adherence to hospice level of care criteria. They lead the interdisciplinary care group (IDG) to develop and implement patient Plans of Care, coordinate services, administer medications, and manage documentation in the electronic health record. The role also includes supervising Home Health Aides and licensed practical nurses, facilitating discharge planning, and orienting new staff.
