About The Position
The Registered Nurse Case Manager/Liaison at AmeriCare Medical is responsible for facilitating the transition of patients from hospital care to home health services. This role involves advocating for patients, educating hospital staff and families about post-acute care options, and managing relationships with hospital personnel to promote the company's services. The ideal candidate will have a strong understanding of the patient discharge process, particularly in relation to Workers' Compensation and Michigan auto no-fault laws.
