About The Position
Ascension at Home, Together with Compassus, is seeking a Registered Nurse Case Manager for Home Health. This role is crucial for supporting patient recovery and enabling them to remain in their homes, allowing them to spend quality time with loved ones and engage in activities they enjoy. The Registered Nurse will empower patients and their families to achieve healthcare goals by educating them on disease management. Additionally, the RN will case manage and provide clinical oversight to an interdisciplinary team. Requirements
- RN license in the state you work
- Two years of experience as a registered nurse
- Current driver’s license
- Current CPR and BCLS certification
