As an RN Case Manager on our team, you will contribute to coordinating high-quality, patient-centered care, collaborating with interdisciplinary teams, and supporting patients and families through every stage of their healthcare journey.

Requirements

  • Current Registered Nurse (RN) license in the state of Indiana.
  • Minimum of 1–2 years of nursing experience (home health or acute care preferred).
  • Strong clinical assessment, critical thinking, and problem-solving skills.
  • Excellent organizational and communication skills.
  • Ability to work independently and manage a caseload effectively.
  • Valid driver’s license, reliable transportation, and proof of auto insurance.

Nice To Haves

  • Knowledge of OASIS documentation and Medicare home health regulations preferred.

Responsibilities

  • Perform comprehensive patient assessments and complete all required OASIS.
  • Develop, implement, and manage individualized Plans of Care in collaboration with physicians and the interdisciplinary team.
  • Provide skilled nursing care and serve as the primary case manager, coordinating services, monitoring patient progress, and communicating changes promptly.
  • Educate patients and caregivers on health conditions, safety, and self-care.
  • Ensure accurate, timely documentation and compliance with Medicare, state, and agency requirements.
  • Participate in case conferences and quality initiatives.
  • Provide on-call coverage, including evenings and weekends, as assigned.
