About The Position
The RN Case Manager position at Boca Raton Regional Hospital is designed to support the physician and interdisciplinary team in facilitating patient care. The primary objective is to enhance the quality of clinical outcomes and patient satisfaction while managing the cost of care. This role involves integrating and coordinating care facilitation and discharge planning functions, working closely with the Utilization Review (UR) RN and Social Work Case Manager. The RN Case Manager is responsible for a designated patient caseload, ensuring effective planning to meet patient needs, manage length of stay (LOS), and promote efficient resource utilization.
