About The Position

The Registered Nurse- Case Management- Float falls under the clinical pathway, as a direct care nurse, and reports to the Clinical Manager, Patient Care Services Service Line The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers. This is an approach to healthcare that empowers and equips people to take charge of their health and well-being and live their lives to the fullest. Requirements

  • 1 full and unrestricted license from any US State or territory

Responsibilities

  • Possess the ability to cross-cover all the necessary clinic areas and other responsibilities as assigned.
  • Provides cover for anticipated and unanticipated absences of Case Managers
  • Acts as a consultant in specialty clinical practice
  • Identifies patient care needs in accordance with the established standards, policies, and procedures consistent with nursing practice
  • Processes referrals for specialty patient care management, including screening for referral to the correct specialty service
  • Coordinates nursing care across the continuum, ensuring seamless transitions in care from outpatient through post-surgical or medical discharge
  • Follow up on referrals, labs, and other tests to ensure continuity of care.
  • Administers medications and procedures per established policies and guidelines.
  • Works with the Operating Room (OR) to ensure that surgical cases are scheduled appropriately
  • Influences care outcomes by collaborating with members of the interdisciplinary team.
  • Works cooperatively as a member of a team in all assignments.
