About The Position

The RN Care Manager at Somatus plays a vital role in the care team, focusing on improving patient outcomes for individuals with chronic kidney disease. This position involves conducting comprehensive assessments, creating and implementing care plans, and coordinating care across various settings. The RN Care Manager will work closely with patients, their families, and healthcare providers to promote self-management, facilitate transitions of care, and provide education on renal health. The role operates in a hybrid telehealth environment, combining remote work with home visits as needed.

Requirements

  • 1+ years of nursing experience in case management or care management, preferably across multiple settings.
  • 2+ years of healthcare-related experience.
  • Current, unrestricted compact Registered Nurse license.
  • BLS certification from a licensed AHA or American Red Cross training facility.
  • Core values consistent with a patient-centered approach to care.
  • Proactive patient advocacy and problem-solving skills.
  • Knowledge and experience in empowering patients in self-management and shared decision-making.
  • Collaborative team player with effective communication skills.
  • Strong analytical and critical thinking skills.
  • Ability to travel throughout the assigned region and conduct home visits as needed.

  • Experience with vulnerable patient populations (ESRD, geriatrics, minorities, low income, uninsured).

Responsibilities

  • Conduct comprehensive assessments of patients' medical, behavioral, pharmaceutical, and social needs, identifying gaps in care and barriers to health.
  • Create and implement care plans in collaboration with patients, nephrologists, PCPs, and other care team members.
  • Coordinate care by serving as an advocate and resource for patients and their families.
  • Facilitate care across various settings, including home, hospital, and skilled nursing facilities.
  • Manage patients during transitions of care to minimize avoidable readmissions.
  • Educate patients on their renal condition and provide self-management support.
  • Provide ongoing reassessment and follow-up to improve patient outcomes.
  • Oversee non-licensed support team members and delegate tasks appropriately.
