About The Position

The Care Manager, Emergency Department RN serves as a patient advocate/liaison, taking a proactive role by early intervention and collaborative practice with staff nurses, physicians, ancillary services, payer sources, and outside agencies assuring appropriate use of resources while focusing on positive outcomes. Works with payers and gatekeepers to assure necessary precertifications / authorizations are obtained. This position must be able to demonstrate the knowledge and skills necessary to provide care and service appropriate to the age of the patients served in his/her area.

Requirements

  • Associate's RN Degree in Nursing required
  • Two years clinical experience (as RN) required
  • Current BLS for the Healthcare Provider completed by 60 days after hire required

Nice To Haves

  • Bachelor's or Master's Degree in Nursing preferred
  • 5 years of Case Management experience in a health care setting preferred
  • Experience with 3rd party reimbursement preferred

Responsibilities

  • Adheres to the Nursing Process to promote safe, quality, cost-effective care by maintaining the standards of practice for Case Management by adhering to the policies and procedures of Holland Hospital and the Care Management Department.
  • Achieves all initial and ongoing competencies and department specific educational requirements.
  • Assesses/screens patients prior to and after admission utilizing a collaborative effort to determine actual and potential health care needs related to the physical, psychological, social, spiritual, financial, behavioral, cultural and educational requirements.
  • Demonstrates ability to analyze collected data to determine the clients appropriate criteria status in addition to identifying those patients who would require further discharge planning interventions.
  • Effectively and accurately documents and communicates assessment data.
  • In a timely manner develops an individualized discharge plan of care based on identified risk factors, by collaborating with other members of the health care team along with the patient and family as appropriate.

  • The Care Manager will actively facilitate communication by collaborating with the patient's Primary Care Provider, UR team, other members of the health care team, along with any community resources to ensure informed decision making, while streamlining the patient care delivery process to avoid duplication or fragmentation.
  • Demonstrates critical thinking skills to initiate available educational opportunities to decrease high risk behaviors.
  • Effectively and accurately documents the above.
  • The Care Manager acts as a patient advocate by supporting the patient/family to encourage self-determination, (realizing that the patients informed decision may not be in their best interest as determined by other health team members).
  • Other Patient Advocate activities would include, early Case Management, MSW, Community Placement Liaison and Hospice, etc. referrals, securing of necessary funding, advocating for appropriate treatment options and alternatives and appealing denials/readmissions on the patient's behalf.
