Join Homewatch CareGivers of Woodbridge and bring your clinical expertise—and your heart—to every family we serve. As our RN Care Manager, you’ll design person-centered plans of care, mentor caregivers, and champion quality that keeps seniors safe, healthy, and comfortable at home.

Requirements

• Active Registered Nurse (RN) license in NJ
• Bachelor’s in Nursing or related human-services field – OR – 2+ years’ geriatric/home-care experience
• Solid clinical assessment skills and a passion for problem-solving
• CPR/First Aid certification and a valid driver’s license
• Warm, professional communication that builds trust with families and field staff

Responsibilities

• Complete in-home RN assessments; craft individualized Care Plans with client/family approval.
• Orient caregivers to each Care Plan, perform field competency checks, and ensure ongoing compliance.
• Track vitals, condition changes, and interventions in our HIPAA-secure EMR; escalate concerns promptly.
• Coordinate with physicians, therapists, and schedulers to keep care seamless and client-centered.
• Maintain audit-ready client files, collect initial service payments, and uphold state home-care regulations.
