About The Position

The Registered Nurse Care Manager at Legacy Hospice is responsible for planning, coordinating, and providing hospice care in accordance with the patient's care plan. This role involves working closely with patients, families, and interdisciplinary team members to ensure that the needs of patients are met effectively and compassionately. The RN will evaluate the effectiveness of care, consult with physicians, and maintain accurate clinical documentation while participating in quality improvement activities.

Requirements

  • Must be a graduate from an accredited School of Nursing.
  • Must be licensed in the State of Wisconsin as a Registered Nurse.
  • One year of RN experience is required; hospice experience preferred.
  • Excellent assessment and problem-solving skills are necessary.
  • Strong organizational skills and knowledge of case management processes are required.
  • Must possess a valid driver's license and have an insured automobile in good working order.
  • Ability to communicate effectively, both verbally and in writing.
  • No exceptions to RN license; must be able to administer pharmaceuticals and perform nursing procedures.

Nice To Haves

  • Experience in hospice care is preferred.

Responsibilities

  • Coordinate hospice care by collaborating with team members and patients/family members.
  • Evaluate the effectiveness of hospice services on an ongoing basis.
  • Perform evaluations for hospice care and conduct informational visits as needed.
  • Utilize the nursing process to provide professional nursing care.
  • Consult with the attending physician and Hospice Medical Director regarding changes in patients' conditions.
  • Coordinate patient services with other members of the IDG and contracted services as needed.
  • Submit accurate and relevant clinical notes in a timely manner.
  • Participate in IDG and case conferences.
  • Ensure patients' plans of care are kept current.
  • Perform on-call duties as required.
  • Actively participate in QAPI meetings and activities.
  • Provide education to patients and family members regarding the dying process and healthcare decisions.
  • Conduct comprehensive assessments of patients and develop initial care plans.
