About The Position

The LTC Charles S. Kettles VA Medical Center is seeking a Registered Nurse (RN) to work as the Care Coordinator for the Ophthalmology Clinic, within the Surgery Service, located in Ann Arbor, Michigan. The Surgery Service RN Care Coordinator functions in the surgery clinic and works to coordinate pre and post-operative care for surgical patients in their surgery section.

Requirements

  • U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
  • Selective Service Registration is required for males born after 12/31/1959.
  • Must pass pre-employment physical examination.
  • Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
  • Participation in the Coronavirus Disease 2019 (COVID-19) vaccination program is a requirement for all Veterans Health Administration Health Care Personnel (HCP).
  • English Language Proficiency.
  • Graduate of a school of professional nursing approved by the appropriate accrediting agency.
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth of the United States.

Responsibilities

  • Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care.
  • Administers medications and procedures per established policies and guidelines.
  • Influences care outcomes by collaborating with members of the interdisciplinary team.
  • Provides patient care, care coordination, health promotion, health education and coaching on wellness, disease prevention, and chronic care management.
  • Assists in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization.
  • Provides peers with informal constructive feedback for improvement.
  • Ensures that appropriate documentation reflects patient assessments and interdisciplinary treatment plans in preparation for surgery.
  • Demonstrates leadership in delivering and improving holistic care to adult and geriatric patients, through collaborative strategies, in compliance with the Michigan Nurse Practice Act, Medical Center, and VAHQ standards of practice.
  • Provides direct nursing care to individual and groups of patients under the immediate guidance and direction of the Surgery Service Nurse Practitioners and ultimately the Chief, Surgery Service.
  • Coordinates changes in the surgery schedule with the patient and Operating Room (OR) staff; ensure timely History and Physicals; organize lodging/admission consults, coordinate surgical equipment/prosthetics/representatives for cases to reduce cancellations and increase OR utilization.
  • Provides pre-operative education to surgical patients as directed by the Surgery Nurse Practitioners (NPs) and Chief of their assigned Surgery Section; coordinate multiple consult/appointment follow ups as needed and any other duties assigned by the supervisor.
