About The Position

The position is a salaried nursing role at Nationwide Children’s Hospital, requiring 40 hours of work per week from Monday to Friday, 8 am to 5 pm. The role focuses on ensuring quality patient care through care coordination, case management, and utilization management of inpatient admissions and transitions of care. The nurse will collaborate with healthcare teams, develop individualized care plans, and facilitate continuity of care post-discharge. The position emphasizes professional nursing practices and patient advocacy in line with the ANA Code of Ethics for Nurses. A current Ohio Nursing License is mandatory.

Requirements

  • BSN required.
  • Current licensure as a registered nurse in the state of Ohio.
  • Valid Ohio driver’s license and proof of auto insurance.
  • Must pass motor vehicle background inspection and maintain qualification of insurance guidelines.
  • Two years of nursing experience required.
  • At least one year in pediatric nursing highly preferred.

Nice To Haves

  • Pediatric certification.
  • Advanced-Continuity of Care Certification (A-CCC) or other Case Management certification.
  • Utilization review and/or discharge planning experience desirable.

Responsibilities

  • Screens and identifies patients.
  • Assesses patient/family strengths and needs.
  • Develops a family-centered plan of care.
  • Facilitates communication and collaboration amongst the health care team.
  • Provides monitoring and follow-up across the continuum of care.
  • Develops patients’ self-management skills through education and resource provision.
  • Manages transitions between settings, caregivers, and providers.
  • Maintains knowledge of current concepts, researches needs and research strategies related to care coordination including case management, utilization review, and discharge planning.
  • Assists in educating others regarding the role of care coordination/case management.
  • Participates in ongoing education related to information technology requirements for the department.
