About The Position

As a Registered Nurse (RN) at Genesis Home Care, you will play a crucial role in enhancing the quality of life for our patients by providing compassionate, one-on-one care in their preferred environment. Your daily work will not only impact the lives of our patients but also their families, as you will interact with them throughout the recovery journey. In this role, you will provide comprehensive skilled care as ordered by the attending physician within a specified geographical territory. You will work autonomously, managing your time efficiently while adhering to the practices and duties outlined by the state. Your responsibilities will include observing and monitoring patient conditions, performing OASIS assessments, and developing individualized care plans that adapt to changing needs. You will administer medications as prescribed and prioritize visits for high-risk patients to help decrease re-hospitalizations. Education will also be a key component of your role, as you will teach patients and their families about disease processes using effective communication methods. You will be responsible for completing all clinical documentation in accordance with agency protocols and Medicare/Federal guidelines, collaborating with the interdisciplinary team and physicians to ensure optimal care is provided. Understanding and following agency policies, procedures, rules, and regulations will be essential, as will communicating any changes in your schedule or availability to schedulers or supervisors.
