Northern Pines Mental Health Center is seeking a Registered Nurse (RN) to join our organization. This full-time role will share time between our Outpatient Psychiatric and Safe Harbor - Crisis Residential teams. Essential Functions include: Ensure each client's medication and medical needs are addressed according to regulations while at the facility Develop and implement appropriate prompt responses to medical emergencies, including intoxication and withdrawal Facilitate the nursing review portion of the weekly team meeting. Provide consultation and advice concerning the health and medical care of clients Responsible for ensuring staff are trained, supervised in regard to medical care in order to complete delegated tasks (schedule annual CPR/AED training for staff, training unlicensed assistive personnel (UAP) at hire, yearly medication administration education for UAPs as well as ongoing education, and continuous review of nursing policies and procedures) Review, complete medication reconciliation of client medications upon arrival to facility in accordance of regulations; assist clients with obtaining medication refills as needed Manage Medication Administration Record (MAR) with UAP assistance, complete MAR audits for each client Ensure medications are stored safely, and in accordance with regulations Complete a Nursing Assessment on clients within 72 hours of admission to facility, including a review of medication side effects and addressing any special needs of the client. Routinely educate, assess, monitor, and update each client's self-administration of medications, including readiness and compliance. Assist the client in developing the necessary skills to administer their medications safely. Routinely assess individuals for medication side effects and medication interactions During shift, the RN is the primary staff administering medications with UAP assistance as needed. The RN will make referrals to and coordinate with community supports which may include primary care, psychiatric services, eye care, and dental care. During shift, the RN is the primary staff to assist bringing clients to medical appointments to improve continuity of care. The RN will be under the clinical supervision of the APRN to routinely address updates to medical policies and procedures, along with reviewing medical and medication concerns. Complete preliminary screenings/ risk assessments with clients Triage with clients as needed. Schedule rapid-access appointments for clients determined to be eligible Assist clients by providing resource and referral Maintain excellent communication and collaboration with all programs within Northern Pines Mental Health Center Communicate with internal and external providers to ensure continuity of care Provide health and wellness education to clients when requested Maintain program tracking of integrated treatment planning by communicating with treatment teams Coordinate/attend multi-disciplinary meetings to address client needs Maintain accurate and timely documentation Measure and record clients' vital signs, such as height, weight, temperature, blood pressure, pulse, and O2 saturation for psychiatry clients Assist with medication refills and start prior authorizations for psychiatry providers

Requirements

  • Must hold a current, unrestricted Registered Nursing License in the state of Minnesota
  • Must qualify as a behavioral health practitioner as defined by the Minnesota Department of Human Services in one of the following ways: Has at least 2,000 hours (approximately two years working full-time) of supervised work experience in the delivery of mental health services. OR Holds a master's or other graduate degree in behavioral sciences or related fields. OR Has completed at least 30 semester hours or 45 quarter hours of coursework in Behavioral Sciences or related fields AND has completed a practicum or internship that required direct interaction with adults or children served, and was focused on behavioral sciences or related fields.
  • Current CPR and First Aid Certification or willingness to train
  • This position requires flexibility in scheduling as Safe Harbor is a 24/7 facility.
  • This position involves travel; a valid Driver’s License and ability to operate a motor vehicle is required
