If so, Aspire Health and Community Services invites you to apply for our Adult Community Clinical Services (ACCS) Team. This position is an excellent opportunity for new nurses who are passionate about supporting adults on their path to independence. In this role, you’ll work in a collaborative and supportive environment where you can strengthen your clinical knowledge. We are seeking a mission-driven RN who is eager to grow professionally, develop strong community nursing skills, and contribute to a program dedicated to empowering individuals and improving lives.

Requirements

  • Graduate of an accredited Nursing program (Diploma/ASN/BSN)
  • Current RN license, CPR, and First Aid certifications
  • Interdisciplinary collaboration experience
  • Strong communication, documentation, and teamwork skills
  • Valid driver’s license and personal vehicle required for client transportation

Nice To Haves

  • Experience working with individuals with mental illness, preferred.

Responsibilities

  • Assess the community regularly to engage and provide resources to clients in their houses
  • Assess health needs and advocate for essential medical services
  • Collaborate with medical providers to implement treatment plans
  • Ensure MAP compliance and complete regular medication audits
  • Provide first aid, emergency responses, and direct nursing care
  • Train, support, and consult with residential staff in residential group homes
  • Participate in Recovery Planning Team meetings and interdisciplinary care
