About The Position

The primary purpose of your job position is to conduct and coordinate the development and completion of the resident assessment to ensure the services they receive are appropriate and effective. This position will also be responsible for evaluating and identifying changes in our residents’ conditions and initiating appropriate steps. It is highly important that this position ensures the facility is in compliance with Medicare and Medicaid programming at all times, along with always meeting appropriate timing for all deadlines. This position will have oversight of all MDS Assessment sign-off’s.

  • Must be licensed as a Registered Nurse in the Commonwealth of PA
  • Basic - word processing, spreadsheet, Internet usage for research/resources

Responsibilities

  • Conduct and coordinate the development and completion of the resident assessment (MDS) in accordance with current regulations and guidelines that govern the resident assessment.
  • Develop preliminary and comprehensive assessments of the nursing needs of each resident, utilizing the forms required by current rules and regulations and facility policies.
  • Certify completion of OBRA (Admission, Quarterly, Annual and Significant change) and PPS assessments by signature and date.
  • Ensure that all members of the assessment team are aware of the importance of completeness and accuracy in their assessment functions and are aware of the penalties for false certification. Hold them accountable for accuracy and completion.
  • Compile data from nursing staff and all Care Team Members to determine the need for resident reassessment.
  • Coordinate the development of a written plan of care (preliminary and comprehensive) for each resident that identifies the problems/needs of the resident, indicates the care to be given, goals to be accomplished, and which professional service is responsible for each element of care.
  • Ensure that the care plan includes measurable objectives and timetables to meet the resident’s medical, nursing, mental, spiritual, and psychosocial needs as identified in the resident’s assessment: and ensure that the care plans are updated and current.
  • Monitor regulatory compliance of specified completion dates of all assessment and care plans.
  • Assist the nursing staff in encouraging the resident and resident’s family to participate in the development and review of the resident’s plan of care.
  • Ensure that all nursing staff are aware of changes made to the care plan.
  • Develop weekly Plan of Care schedule and submit notices of additions or amendments to the schedule.
  • Review resident medical records to ensure the plans of care address resident needs or concerns and ensure interventions to address these are implemented.
  • Assist Activity Staff in developing activities required for the resident assessment and plan of care.
  • Ensure that OBRA and PPS (scheduled and unscheduled) resident assessments and care plan reviews are made on a timely basis.
  • Coordinate scheduling of MDS assessments with services provided in order to capture the most appropriate RUG level that reflects the resident’s condition.
  • Coordinate the review and revision of the resident’s care plan by the interdisciplinary team after each quarterly review or other assessment, assuring that the care plan is evaluated and revised each time an assessment is done or when there is a change in the resident’s status.
  • Develop, implement and maintain an ongoing quality assurance program of the MDS/Care Planning process.
  • Participate in planning, developing, and implementing Restorative Nursing Programs (ROM, exercise, walking, continence, turning/positioning, etc.).
  • Participate in resident discharge planning as appropriate and necessary.
  • Utilize the most updated version of the RAI manual (Resident Assessment Instrument) for the accurate initiation and completion of the resident MDS Assessments as required by OBRA and PPS regulations.
  • Perform administrative or other duties deemed necessary by Director of Nursing or Administrator.
  • Act as Chairperson (or co-chair) of the Interdisciplinary Care Plan Conference.
  • Work with Care Plan Team to develop an interdisciplinary comprehensive assessment for all residents.
  • Review assessments of interdisciplinary team to ensure completion and proper interventions for identified problems.
  • Participate in Quality Assurance and Performance Improvement Subcommittee projects to affect problem identification and solution-finding by contributing to data collection and interpretation.
  • Review resident pre-admission information to determine projected RUG level and/or appropriateness of long-term care placement.
  • Conduct or coordinate the interviewing of each resident prior to the completion of the MDS.
  • Evaluate each resident’s condition and pertinent medical data to determine the need to amend or revise the plan of care.
  • Interview staff as necessary to ensure accuracy of MDS coding and compile data from direct care nursing staff, RN Supervisors, and other Care Team members to determine the need for resident reassessment.
  • Attending resident applications/pre-admission conferences to provide preliminary Case Mix Data and to contribute to the initiation of the resident plan of care.
  • Monitor resident Case Mix Index and facility CMI to ensure accuracy and to determine warranted interventions to update or correct the report.
  • Coordinate periodic (annual or as needed) tracker training for nursing staff to ensure staff have an adequate understanding of documentation expected to support MDS assessments.
  • Educate Interdisciplinary Care Team to update information regarding regulatory requirements or changes that affect the MDS Assessment process.
  • Develop and initiate orientation program for RN’s to MDS, Care Planning process and RNAC positions as necessary.
  • Respond to fire alarms as required (refer to Fire Safety Manual for employees’ responsibilities and those that are specific to your position.
  • Treat all residents fairly, with kindness, dignity and respect.
  • Maintain confidentiality of all resident and facility information.
  • Report all complaints, concerns made by the resident, allegations of abuse or neglect to direct supervisor or the RN Supervisor.
  • Follow all safety procedures and precautions in the performance of your duties and when operating equipment.
  • Report all safety violations, unsafe or hazardous conditions and any equipment issues immediately.
  • Review Corporate Compliance educational materials, comply with Corporate Compliance directives, and report any Corporate Compliance issues according to established procedures.
  • Recognize your role as part of the QAPI efforts of the organization. Attend trainings to build understand and capacity to undertake QAPI work. Carry out QAPI roles and responsibilities as assigned. Follow established policies and procedures in support of QAPI efforts.
