About The Position

"Navitas Healthcare, LLC" is seeking a dedicated Registered Nurse (RN) for an Adult Care Home Care role. This position involves delivering high-quality, patient-centered care in patients’ homes, ensuring optimal recovery and health outcomes through comprehensive nursing practices.

Requirements

• Minimum 2 years of Med/Surg experience (acute or post-acute setting)
• Recent experience within the past 1 year required
• Home care nursing experience required
• Strong clinical assessment and patient care skills
• Associate Degree in Nursing (ADN) required
• Active New York State RN License required
• BLS (AHA) certification required
• Familiarity with EMR systems (Epic preferred)
• Ability to work independently in a home care environment

Responsibilities

• Assess, plan, implement, and evaluate individualized patient care plans
• Provide direct care to pre- and post-surgical patients in a home setting
• Administer medications and monitor patient responses
• Perform wound care, IV therapy, and infection control procedures
• Monitor vital signs and report abnormalities
• Educate patients and families on care plans and post-treatment instructions
• Maintain accurate documentation in EMR systems
• Collaborate with interdisciplinary teams for care coordination and discharge planning
