Requirements
- BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
- Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.
- Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.
Responsibilities
- Lead the implementation of personalized care plans to ensure every patient receives treatment tailored to their specific recovery goals.
- Monitor and translate complex patient data into actionable communication, keeping the broader care team and family members informed and aligned.
- Coordinate multidisciplinary teams as the primary point of contact to ensure seamless, high-quality care transitions throughout the patient’s stay.
- Execute clinical orders with precision and empathy, maintaining the highest standards of safety and organizational excellence.
- Champion the discharge planning process by collaborating with the healthcare team to ensure patients are empowered and prepared for their next stage of healing.
