As a Registered Nurse (RN), you will travel into the community to provide essential nursing services across various settings, including residential care facilities and outpatient clinics. We are looking for individuals who are dedicated, skilled, and passionate about helping others. Your expertise will play a crucial role in evaluating and coordinating care for individuals facing behavioral health challenges. In this role, you will be responsible for performing nursing assessments, monitoring health status, and collaborating with treatment teams out in the community to develop action plans tailored to each client's needs. Your clinical competence will ensure that clients receive the highest quality of care in a variety of settings.

  • Perform nursing assessments and evaluate client/patient physical conditions to determine service needs.
  • Create and implement action plans based on clients' physical and mental health treatment needs.
  • Monitor health status and coordinate care with the treatment team to ensure comprehensive support.
  • Administer medications and oversee compliance with state regulatory standards.
  • Provide health education and disease prevention strategies to clients and their families.
  • Arrange medical appointments and manage emergencies, advocating for necessary medical services.
  • Maintain accurate and timely documentation in accordance with company policies.
  • Current Registered Nurse licensed in the state of practice.
  • Must be willing to become licensed in additional states.
  • Must be comfortable providing telehealth services.
  • BCLS certification required; ACLS certification preferred.
  • Successful completion of background check including criminal record, driving record, abuse/neglect, and fingerprint check.
  • Current driver's license, acceptable driving record, and current auto insurance.
