About The Position

Kingsboro Psychiatric Center is recruiting a Registered Nurse 2 Community to work with individuals with mental illness to prepare them for discharge to a skilled nursing facility. As Registered Nurse 2 Community at Kingsboro Psychiatric Center, you will be part of the Discharge Planning Team as the nurse expert. Duties will include coordinating meetings with family members to address barriers related to referrals and admissions of individuals to skilled nursing facilities, developing and overseeing agreements between Kingsboro Psychiatric Center and skilled nursing facilities to provide convalescent care, conducting site visits to skilled nursing facilities, coordinating treatment, and developing individualized care plans, and identifying other supports and personal items to assist the individual living at the skilled nursing facility.

Requirements

  • License and current registration as a registered professional nurse in New York State.
  • 2 years of post-licensure clinical nursing experience; 1 year of this experience must have been providing direct services to individuals diagnosed with mental illness, developmental disabilities, addiction, or substance use disorders in a community setting.
  • A Bachelor's degree in nursing can substitute for 1 year of post-licensure clinical nursing experience.
  • A Master's degree in nursing can substitute for 2 years of post-licensure clinical nursing experience.
  • Candidates from within State service may be considered for transfer to a competitive class position if they have one year of permanent competitive or 55-b/55-c service in a title eligible for transfer under Section 70.1 of the Civil Service Law.
