About The Position

The position is responsible for the coordination of care to meet the identified needs of the patient and family. The role involves supervising the management of patient care utilizing critical thinking and the nursing process, discharge planning, resource identification, linking and scheduling patients with required services, monitoring care delivery, advocating for the patient, care planning, and evaluating outcomes. The position requires coordination of patient care across the healthcare continuum and assisting other team members with meeting individual patient needs. Flexibility of hours may be required.

Requirements

  • Bachelor’s degree (BSN) required; Associate Degree in Nursing (ADN) will be considered if enrolled in a BSN program with completion within 2 years.
  • Minimum 1 year of current experience in acute care area; 2 years preferred.
  • Critical Care unit and Trauma nursing experience preferred.
  • Must pass Basic Dysrhythmia course within 21 days of hire.
  • Basic computer skills and experience with computerized documentation systems required.
  • Current unrestricted Registered Nurse (RN) license issued by California Board of Registered Nursing (BRN).
  • Current Basic Life Support Provider (BLS) and Advanced Cardiovascular Life Support Provider (ACLS) certifications.
  • Current National Institutes of Health Stroke Scale (NIHSS) certification within 30 days of hire/transfer.

Nice To Haves

  • Experience in critical care nursing.
  • Proficiency in delivering age-specific care to adolescent, adult, and geriatric populations.

Responsibilities
